Medicare Enrollment Guide for Upstate New York

Turning 65 or becoming eligible for Medicare can feel overwhelming. Understanding your options and enrollment periods is crucial to ensuring you have the coverage you need. This guide will help Upstate New York residents navigate the Medicare enrollment process.

Understanding Medicare Parts

Medicare Part A (Hospital Insurance)

Part A covers inpatient hospital stays, skilled nursing facility care, hospice care, and some home health care. Most people don't pay a premium for Part A if they or their spouse paid Medicare taxes while working.

Medicare Part B (Medical Insurance)

Part B covers doctor visits, outpatient care, preventive services, medical equipment, and some home health care. There is a monthly premium for Part B, which varies based on your income.

Medicare Part C (Medicare Advantage)

Medicare Advantage plans are offered by private insurance companies approved by Medicare. They include all Part A and Part B benefits, and often include prescription drug coverage (Part D) and additional benefits like dental, vision, and hearing.

Medicare Part D (Prescription Drug Coverage)

Part D helps cover the cost of prescription drugs. It's offered by private insurance companies and can be added to Original Medicare or some Medicare Advantage plans.

Medicare Supplement Insurance (Medigap)

Medicare Supplement plans help pay for costs that Original Medicare doesn't cover, such as copayments, coinsurance, and deductibles. In New York, Medigap policies are standardized and lettered (Plan A, B, C, D, F, G, K, L, M, and N).

Important Enrollment Periods

  • Initial Enrollment Period: A 7-month period that begins 3 months before you turn 65, includes your birth month, and ends 3 months after your birth month.
  • Annual Enrollment Period: October 15 - December 7 each year. You can change Medicare Advantage or Part D plans during this time.
  • Medicare Advantage Open Enrollment: January 1 - March 31. If you have a Medicare Advantage plan, you can switch to another Medicare Advantage plan or return to Original Medicare.
  • Special Enrollment Periods: Triggered by certain life events like moving, losing employer coverage, or qualifying for Medicaid.

New York offers some unique protections for Medicare beneficiaries:

  • Year-round guaranteed issue rights for Medigap policies — in New York, insurers cannot deny you a Medigap policy or charge more based on your health status at any time, not just during the initial federal enrollment window
  • EPIC (Elderly Pharmaceutical Insurance Coverage) program to help with prescription drug costs
  • Medicare Savings Programs to help pay Medicare premiums
